Indicate on the application if this is a new member or renewal application.  Also check your method of payment. 

To pay by check: 

Make check payable to:  The Larkspur Chamber.  Then after completing the application, print it and send it with payment to:

You may also fax the application to 510 – 868 – 8550

To pay using PayPal:
